If my income goes up during the year, do I lose Extra Help right away?

Short answer: No. Extra Help lasts through December 31 even if income changes mid-year. People who get it automatically through Medicaid, a Savings Program or SSI keep it through year end, and through the next year too if they qualified between July 1 and December 31.

Medicare.gov says an Extra Help approval runs for the rest of the calendar year; a mid-year change in income doesn’t end it before December 31. If the person still meets the limits for the next year, it continues. Medicare or Social Security sends a notice only if the person no longer qualifies or is being moved to a different plan; no notice means Extra Help and the plan continue.

For people who get Extra Help automatically (full Medicaid, QMB, SLMB, QI or SSI), 42 CFR 423.773(c)(2) sets the minimum period:

  • Deemed eligible January 1 through June 30: through the end of that year.
  • Deemed eligible July 1 through December 31: through the end of that year and the whole following year.

A person told they won’t qualify next year gets a special enrollment period to change Part D or Medicare Advantage coverage.
