Does my plan cover weight-loss drugs like Ozempic or Wegovy?

Short answer: Sometimes. Coverage of GLP-1 drugs for weight loss varies by employer and plan and is often limited or excluded due to cost. In 2025, about 19% of firms with 200+ workers (and 43% of those with 5,000+) covered them for weight loss. Original Medicare does not cover them for weight loss alone.

GLP-1 medications such as semaglutide (Ozempic, Wegovy) and tirzepatide (Mounjaro, Zepbound) are often covered when prescribed for type 2 diabetes, but coverage for weight loss is far less consistent. Because of high cost and rapidly growing use, many employers limit or exclude the weight-loss indication, sometimes requiring prior authorization, a body-mass-index threshold, or participation in a lifestyle program.

Survey data show how varied this is: in 2025 roughly 19% of firms with 200 or more workers, and about 43% of firms with 5,000 or more workers, covered GLP-1s for weight loss, and coverage decisions are shifting year to year. Original Medicare does not cover drugs used only for weight loss, though it may cover a GLP-1 for an approved condition such as diabetes or to reduce cardiovascular risk.

Check your plan’s formulary and any prior-authorization rules before assuming coverage. If a drug is not covered for weight loss, ask whether it is covered for a related diagnosis, whether an exception is possible, and what manufacturer savings programs may apply.
